INFORMATIONAL MEETING SCHEDULING:

For advisory/ collaborations:

Up to 1 hour Zoom meeting with Dr Creado to understand your organizations vision, mission, needs. Dr Creado will also describe the need for a cohesive sleep strategy in your space, along with data points, statistics, gap analyses and more. 

Complete the box below, and it will be emailed to our team.

You will then receive an email regarding pricing (flat fee) or whether we choose to waive the fee.

NDAs can be signed (one or two way) as deemed appropriate.

Once payment is received and NDAs are signed, you will be provided possible appointment times.

You will be able to reschedule free of cost more than 48 hours prior to the appointment, otherwise you will be charged for the meeting.

If you are interested in any of the services I offer, you can get more information on a Zoom or phone call. This will give you the opportunity to explain your unique situation, and learn more about how I may be able to help you, before you commit to a consultation. 

CONSULTATION SCHEDULING:

You will receive a document on FAQs (frequently asked questions).

You will also be provided with our treatment contract that you will sign and return to us.

You can then reply to us if you want to go ahead with the evaluation.

You will be sent information on payment and scheduling (Zoom or phone).

You are required to complete payment, after which you will be provided possible appointment times.

Once payment is received and appointment time confirmed, you will receive documents with instructions on what to fill out and return to us PRIOR to the scheduled appointment.
